Included in the family folder this week are the following forms that need to be filled out:

Basic Criminal Background Check

This form is for anyone who might volunteer here at school in any way.

Confidential Employee and Volunteer Questionnaire

This form is also for anyone who might volunteer here at school in any way. 

Field Trip Driver Form

This form is for anyone who might drive for a field trip. 

Safe Environment Form

Please read this form over carefully and fill it out accordingly.  IF you have any questions, please call Mrs. Pyka at school.  It is mandatory that this information be collected for each family.

Magazine Sale

During the period between Sept. 9th and Sept. 30th, our students will be conducting a magazine subscription campaign under the auspices of QSP, Inc.  You may purchase new, renewal or gift subscriptions of your favorite magazines.  Choose from over 600 titles and SAVE UP TO 85% OFF THE COVER PRICE!  Our school earns 40% if all magazine sales.

Parents, you may also send emails to distant friends and family to shop on-line at www.qsp.com.  They will need the St. Boniface School ID # 425007559 if they want to order on-line; this will ensure the school gets the credit from their purchase.

Free and Reduced Lunch Applications

If you have not filled out your Free and Reduced Lunch Application, please do so and return it to school as soon as possible.  If you have any questions regarding your income, please contact Mrs. Pyka.

Canon BCI-3e Ink Cartridges

If anyone could make use of two new boxes of Cannon ink, please contact Mrs. Pyka.  One box has a black ink cartridge and one box has a set of three color ink cartridges.

H1N1 Flu Information

Enclosed in this week's folder is information from Buffalo County.  Please read this information over carefully. 

Please encourage your child to take the proper precautions to avoid spreading germs (hand washing, covering their mouths when they cough or sneeze).

Students who are ill must stay home from school until they are well.  If a student comes to school sick, they will be sent home.

Athletic Shorts

Students are asked to not wear athletic shorts to school with their uniforms.
